Section 0780-01-14-.03 - FILING OF STATEMENTS

Initial statements of beneficial ownership of equity securities required by T.C.A. § 56-3-702 shall be filed on Form 1 attached hereto. Statements of changes in such beneficial ownership required by T.C.A. § 56-3-702 shall be filed on Form 2 attached hereto. All such statements shall be prepared and filed in accordance with the requirements of the applicable form.
