Section 74:32:05:04 - Release report

Release reports shall provide the following information:

(1) The specific location of the release;

(2) The type and amount of product released;

(3) The responsible person's name, address, and telephone number;

(4) An explanation of any response action that was taken;

(5) The list of agencies notified;

(6) The suspected cause of the release;

(7) The date and time of the release to the extent known; and

(8) The immediate known impacts of the release.
