Section 67:62:14:06 - Renewal of eligibility - Fee

A qualified mental health professional shall register with the department for renewal of eligibility. A qualified mental health professional may register for renewal anytime during the year before the qualified mental health professional's current eligibility ends. A renewal registration must include:

(1) The qualified mental health professional's name and address;

(2) The qualified mental health professional's employer, or place of practice, with address and telephone number;

(3) A copy of the qualified mental health professional's South Dakota professional license or certificate; and

(4) The renewal fee, as determined by the department.
