A certificate issued by the department applies only to the applicant agency, the original facilities, and program classifications for which the certificate was issued. The agency shall notify the department in writing within thirty days before a change of ownership, facility, or program for a determination on continued accreditation. A new application for accreditation must be filed if there is a change of ownership, facility, or program.
