The department shall provide written notice to an agency of the department's intent to suspend or revoke the agency's accreditation.
The suspension or revocation is effective fifteen days after receipt of the notice. The notice shall contain the reason for the department's action, the opportunity for the agency to request reconsideration by the department, and the appeal process.
An agency's request for reconsideration must be in writing and received by the department within 15 days of receipt of notification of suspension or revocation.
