Section 67:42:17:35 - Required child recordsA provider shall maintain a record for each child that shall include:
(1) Child's name and date of birth;(2) Parent or guardian's name and telephone number;(3) Emergency contact name and telephone number;(4) Parent permission for emergency medical treatment;(5) Names of individuals authorized to pick up child as needed;(6) Health information including any allergies or special needs;(7) Current immunization record; or name of school for a school-age child;(8) Medication permission as needed;(10) Date of enrollment; and(11) Date enrollment ends. Each required child record shall be reviewed and updated annually and made available to the department for verification of the contents.
