Section 67:40:19:03 - Individual care plan

The department and the individual shall develop an individual care plan for each individual eligible for services based on the individual's assessment. The plan must contain:

(1) The services or supports authorized;

(2) The number of service hours authorized;

(3) The tasks assigned to the provider; and

(4) The amount of any copay, if applicable.
