At least 30 days before the department implements the prior authorization requirements for a particular drug, the department shall provide a written notice to providers. The notice shall contain the following information:
(1) The date on which the department intends to begin requiring prior authorization for the drug;(2) The name or names of alternative, therapeutically equivalent drugs that do not require prior authorization and which may be substituted;(3) A statement that the pharmacist or prescribing medical professional must seek prior approval from the department if the medical professional chooses not to use one of the alternative drugs available; and(4) A statement that the recipient is responsible for the cost of the drug if prior approval is not obtained or a therapeutically equivalent drug is not substituted.S.D.
