The department does not cover the following items or services and, if provided, reimbursement must be obtained from the recipient:
(1) Extended wear or daily disposable contact lenses;(2) Regular eyeglasses or contact lenses when used to supplement another pair of corrective vision lenses;(4) Tinting, additional charges for photochromic lenses, lens decoration, or special lens coating;(5) Frames if a lens change is not medically necessary;(7) Consultation services. The provisions of this rule are not applicable to the items or services provided for children under the age of 21 if documented as medically necessary.
