Section 67:16:02:03.03 - Required modifier codes

A modifier provides the means by which the reporting provider indicates on the claim form that a service or procedure that was performed was altered by some specific circumstance but not changed in its definition or code. If applicable, modifier codes must be included on the provider's claim for services. A list of authorized modifier codes is available on the department's modifier website.
