Section 55:08:03:03 - Election of coverage after initial eligibility period

An eligible employee may elect coverage for the employee and the employees dependents after the initial eligibility period upon showing proof of insurability for each person to be insured and may increase the employees coverage after the initial eligibility period upon showing proof of insurability.
