Laboratory chain of custody begins when the specimen is received from the collection site observer and continues with completion of testing and verification, reporting or results, storage, and final disposition. When the specimen is handled or transferred, the date and purpose must be documented on a chain of custody form; and every individual in the chain of custody must be identified.
