The department shall notify the consumer in writing of the consumer's ineligibility to receive services within 30 days of application or of termination of services at least ten days before termination. The notice shall state the specific rule supporting the action taken and explain the consumer's right to request a fair hearing and to be represented by others at the hearing. The notice shall include the time in which a hearing may be requested and the information needed to make the request. The consumer has 30 days after receiving the notice to request a hearing. Attendant services will continue until the appeal is resolved.
