An interfacility transfer occurs when the individual is transferred from one Medicaid certified swing bed or nursing facility to another, with or without an intervening hospital stay. Interfacility transfers are not subject to PASRR. If an individual transfers from a Medicaid certified swing bed or nursing facility to a hospital or to another Medicaid certified swing bed or nursing facility, the transferring nursing facility is responsible for ensuring that copies of the individual's PASRR findings accompany the individual.
