Section 46:11:05:03 - ISP The ISP shall include documentation of the following:
(1) The participant's vision of a good life including:(a) People, places, experiences, and possessions which bring satisfaction, joy, and contentment to the participant; and(b) Supports which promote the participants health, safety, and role as a valued member of the community;(2) The participant's goals that include:(a) Identification of quality of life outcomes for the participant;(b) Consideration of desired employment outcomes;(c) Evaluation of past and future life experiences which will promote the participants vision for a good life; and(d) Specific action steps which will assist the participant to attain the goal;(3) Consideration of information gathered through person centered discovery;(4) Consideration of integrated supports and resources available to the participant;(5) The type, scope, amount, duration, frequency, and appropriateness of each service;(6) The recommendations from any assessment listed in § 46:11:05:02;(7) A description of how the participant's behavior support plan, if applicable, is implemented in conjunction with the ISP;(8) Team discussion regarding guardianship of the participant, if applicable, along with supports provided to the participant in decision-making and least restrictive options as an alternative to guardianship;(9) The frequency that fiscal reports are provided to the participant, the participant's parent if the participant is under 18 years of age, or the participant's conservator, if any, pursuant to § 46:11:04:13;(10) The estimated amount of time a participant may remain unsupervised, as required in § 46:11:05:15;(11) A statement informing the participant, the participant's parent if the participant is under 18 years of age, or the participant's guardian, advocate, or conservator, if any, how to contact the division with any concerns;(12) A statement informing the participant, the participant's parent if the participant is under 18 years of age, or the participant's guardian, advocate, or conservator, if any, how to report and to whom, any alleged incidents of abuse, neglect, or exploitation;(13) A statement informing the participant, the participant's parent if the participant is under 18 years of age, or the participant's guardian or advocate, if any, of the services available from the CHOICES waiver and a list of all available providers and the right to appeal to the division if choice of services or provider is denied; and(14) A statement to the participant, the participant's parent if the participant is under 18 years of age, or the participant's guardian or advocate, if any, of full disclosure of the potential conflict of interest that exists due to CSPs providing other waiver services and service coordination. This statement is required until such time as the potential conflict of interest does not exist.S.D. Admin. R. 46:11:05:03
