Each facility must have written policies and procedures for all seclusion use. The use of seclusion shall be based on a comprehensive assessment of the patient's behaviors to prevent harm to self and others. Seclusion use requires a physician's, physician assistant's, or nurse practitioner's order including specific time frames and type of seclusion. Continued use of seclusion and reorders may be given only by the physician, physician assistant, or nurse practitioners after a review of the patient's condition. Seclusion may not be used to limit mobility, for convenience of staff, for punishment, or as a substitute for supervision. Seclusion may not hinder evacuation of the patient during fire or cause injury to the patient. Patients in seclusion must be visually monitored by staff at all times. Video surveillance alone of a patient in seclusion is not sufficient to meet the requirement for visual monitoring.
