Section 20:51:19:11 - Forms required for continuing education sponsors

The form for approval of continuing education programs must be completed by the sponsor and submitted to the board. The form must include the following information:

(1) Name of the sponsor and address;

(2) Name of the person in charge;

(3) Location of the program;

(4) Estimated number of the pharmacists and technicians participating;

(5) General title of the program;

(6) Type of program;

(7) How the program objectives will be met;

(8) Estimated contact time;

(9) How attendance or participation will be proven;

(10) How certificates will be awarded; and

(11) A copy of any examination, if utilized.
