Section 20:38:33:08 - Duplicate certificate fees

Duplicate certificate fees are as follows:

(1) Intern, five dollars;

(2) Licensee, fifteen dollars; and

(3) Business entity, fifteen dollars.

These fees may not apply to any duplicate license obtained electronically by the licensee directly from the licensee's profile on the board's database.
