An individual, partnership, or corporation applying for a license from the commission must provide his name or all names of the partnership or corporation, with any nicknames or aliases; a permanent address at which the applicant can receive official correspondence from the commission; a local address; home and business telephone numbers; age, date of birth, place of birth, height, weight, color of hair, color of eyes, sex, marital status, citizenship, social security number, and next of kin; previous racing license history; any rulings made against him, the partnership, or the corporation by any racing jurisdiction; any arrest or criminal history; and any other information that the commission may consider necessary. The information must be provided on an application form provided by the commission. In addition, the commission may require the applicant to provide photographs or fingerprints, taken at a time and place and in a manner required by the commission.
