Section 12:56:05:06 - Contents of application for applicator license

An application for a licensed applicator, licensed operator, and public operator's license must contain the following:

(1) The name of the applicant;

(2) The applicant's partnership or organization and its permanent address;

(3) The local base of operation and maintenance;

(4) The telephone number;

(5) Whether the applicant wishes to be certified as a ground applicator, an aerial applicator, or both; and

(6) The date and the signature of the applicant.
