Rule 8 - Preliminary ApplicationThe Preliminary Application promulgated by the Corporation shall require submission of the following information from each Applicant:
(a) the name, address, and principal contact for the Applicant;(b) state and federal tax identification numbers;(c) the identification of the Qualified Business;(d) a detailed description of the relationship between the Applicant and the Qualified Business that establishes that the Qualified Business, or its Affiliate, supplies goods or services to the Applicant or is a material service provider or material customer of the Applicant, including appropriate documentation to support the description; and(e) any other necessary and relevant information as determined by the Corporation.
