Section 216-RICR-10-10-1.6 - Authorized FormsA. In order to promote and maintain uniformity in the system of vital records, the forms of certificates, records, and other reports required by the Act or these Regulations shall include, as a minimum, the items recommended by the Federal agency responsible for national vital statistics, subject to the approval of and modification by the Director.B. No forms other than those supplied by the State Registrar shall be used for vital records, and all such forms, records, and reports are the property of the State of Rhode Island and shall be surrendered to the State Registrar or his/her representative upon demand.C. The State Registrar shall prepare and issue such instructions concerning the use of forms and supplies as may be required to secure the uniform observance of the Act and the maintenance of an adequate system for the collection, registration and preservation of vital records throughout Rhode Island.216 R.I.
