Section 3490.153 - Information provided to the district attorney and law enforcement officialsThe school official shall provide the following information to the district attorney and law enforcement officials on a form provided by the Department:
(1) The name, age and home address of the student.(2) The name and address of the school.(3) The name and address of the student's parents or guardians.(4) The name and address of the administrator or school employe who made the report.(5) The name, work and home address of the school employe suspected of abusing the student.(6) The nature of the alleged offense.(7) Specific comments or observations that are directly related to the alleged incident and the individuals involved.
