Section 2050.14 - Client filesFor each client except those receiving information and referral service only, providers shall establish and maintain a separate client file consisting of both of the following:
(1) A description of the client's need for service; the particular service or services provided; and pertinent facts, dates and identifying data.(2) Documents pertaining to the determination and redetermination of eligibility, including copies of written notice forms and correspondence, and other documents concerning actions, proposed actions or service requests.
