Section 690:5-5-1 - Notice of proposed hearing Whenever the Board chooses to exercise its statutory authority under Section 1619 of the Speech-Language Pathology and Audiology Licensing Act to conduct a hearing to determine whether suspension or revocation of a license would be ordered or a reprimand issued, the Board shall give at least thirty (30) calendar days written notice of such hearing to the licensee against whom the proceeding is directed. The Notice of the Hearing shall be served on the licensee either personally or by certified or registered mail and shall include the following information:
(1) the time, place, and nature of the hearing;(2) the legal authority and jurisdiction under which the hearing is being conducted;(3) the ground or grounds constituting or forming the basis of the charge(s) against the licensee, including reference to the particular section(s) of the statute and/or rule involved.(4) information regarding the licensee's right to legal counsel Okla.
