Section 165:59-9-17 - Reporting requirements

Each Contributing Provider to the OLF shall, within thirty (30) calendar days from the date of a request made for information or a report by the Administrator or contracted agent, submit to the dministrator or contracted agent a completed report form based on a 12-month period identified by the Administrator or contracted agent, containing such information as designated by the Administrator.
