Section 165:35-45-9 - Renewable energy recycling facility Annual reporting
(a)Submission of annual report. Electronic submission of the annual report is preferred; however, all forms of submission will be accepted.
(b)Contact information. Each renewable energy recycling facility owner shall notify, in writing, the PUD Director within thirty (30) days of a change in the owner's contact information.
(1) The update shall include the name(s) physical street address(es)), electronic mail addresses, and telephone number(s) of the designated individual(s) and shall be furnished applicable to each renewable energy recycling facility so that PUD will be able to reach the responsible person at any time. If this information is unavailable, the renewable energy recycling facility may seek a waiver from the PUD Director by making the request in writing. Each renewable energy recycling facility shall promptly furnish such other information as PUD or the Commission may request, unless otherwise ordered by the Commission.
(2) The contact name(s) provided pursuant to this subsection shall be the individual(s) primarily responsible for:
(A) Providing customer service;
(B) Repair and maintenance;
(C) Answering complaints;
(D) Regulatory matters;
(E) Primary emergency;
(F) After hours emergency;
(G) Annual reporting;
(H) Attorney for regulatory matters; and
(I) Community liaison.
(c)Change in ownership. The owner of a renewable energy recycling facility shall notify the PUD Director in writing within thirty (30) days after a change in ownership.
