Section 165:30-21-18 - Operational records Operational records shall be documents supporting miles traveled in each jurisdiction and the total miles traveled. These documents can be fuel reports, trip sheets, logs or computer runs that can be supported by source documents, when requested. An acceptable source document to verify fleet mileage is some type of Individual Vehicle Distance Record (IVDR). IVDRs should contain the following basic information:
(1) Date of trip (starting and ending);(2) Trip origin and destination;(3) Routes of travel and/or beginning and ending odometer readings;(4) Total trip mileage (including all movement, loaded, empty, deadhead, and/or bobtail miles);(5) Mileage by jurisdiction;(6) Unit number or vehicle identification number;(7) Vehicle fleet number;(8) Registrant's name; and(9) Driver's signature and/or name.Okla.
