Section 33-30-02-01 - Licensure application An application for a license to practice environmental health must be made to the state health officer on forms provided by the officer upon request. The application must contain such information as may be reasonably required.
1. Each application for a license must be accompanied by: b. A transcript verifying completion of college degree. 2. All applications must be signed by the applicant. 3. The state health officer may request such additional information or clarification of information provided in the application as the health officer deems reasonably necessary. 4.
