Section 06N .0116 - [Effective until 2/26/2024] FORM BAR-13

(a) The Form BAR-13 shall be filed when one applies to open or manage a new mobile barber shop. It requires the following:

(1) the name of the shop;

(2) the permanent business address required by G.S. 86B-30(g);

(3) the name and address of the mobile barber shop owner or manager;

(4) if available, the email address, website, or social media handle of the mobile barber shop;

(5) the physical dimensions of the mobile barber shop;

(6) the vehicle identification number, license plate number, and vehicle make and model of the mobile barber shop;

(7) the mobile barber shop business hours;

(8) an explanation of how the mobile barber shop will dispose of sewage and wastewater;

(9) the type of fixtures installed; and

(10) the date the mobile barber shop will be ready for inspection.

(b) The fee required by Rule .0101(a)(22) of this Section shall accompany this form.

(c) The Form BAR-13 shall include the applicant's attestation that the information in the form is correct.
