The school district in which the correctional facility is located shall notify the department, and the school district identified pursuant to Education Law, section 3202(7)(b) as being responsible for the educational costs of such child, that a request for educational services has been received from the correctional facility on behalf of an incarcerated youth. Such notice shall be on a form prescribed by the commissioner and shall be provided within 30 days of initiation of educational services.
