Section 134.6 - Responsibilities of a program participant(a) A program participant: (1) must notify the program in writing of a change in the program participant's contact information at least seven days in advance of the change, or immediately thereafter if such change is necessitated by an emergency or other unforeseen event;(2) must notify the program of any significant change in the information contained in their application; and(3) must return the identification card to the program immediately upon withdrawal or cancellation of participation in the program.N.Y.
