Section 403.4 - Case management
(a) Staff of the social services district shall have basic responsibility for case management, which means those activities necessary for initiating and continuing the provision of services on a case-by-case basis. Such responsibility includes:
(1) determining eligibility for services;
(2) developing a plan of service, consistent with an applicable employability plan developed in accordance with section 385.4(a) of this Title on the basis of an evaluation or reevaluation of the client's situation for the purpose of achieving an identified service goal;
(3) authorizing the scope, type of duration of services to be provided;
(4) assessing the quality and appropriateness of services provided;
(5) maintaining such recipient and services information as required by the department; and
(6) reporting information as required by the department.
(b) These activities shall not be delegated or purchased. When services are purchased by the social services district, the district may request that the provider obtain and transmit to the district the information necessary to perform the case management function. Such an arrangement shall be clearly specified in the purchase of services contract with the provider.
(c) In managing multiple service cases, a strong consideration shall be given to the establishment of a case manager system so as to minimize costly duplication of effort and reduce confusion on the part of the client and the worker.
