Section 343.10 - Reporting

(a) Required reports. Certified providers must prepare and provide the division with statistical, financial, pre and post test format and/or results, and evaluative reports at such times as requested and on forms provided by the division.

(b) Failure to report. Failure to submit such reports may result in termination of certification in accordance with section 343.15 of this Part or denial of renewal of certification in accordance with section 343.14 of this Part.
