having made application for approval to the NEW YORK STATE DRUG ABUSE CONTROL COMMISSION pursuant to the provisions of Mental Hygiene Law, §81.38 to operate a drug abuse treatment and/or preventive education program, and having been found to be in compliance with such law and applicable Rules and Regulations of the Commission, this Certificate of Approval is hereby granted for the operation of a program in conformity with that described in the application for the period ________ to ________.
