Section 8:62-3.6 - Issuance of permit(a) A permit shall be issued by the Commissioner to an applicant who has demonstrated compliance with all of the mandatory requirements of this subchapter.(b) Each permit for a lead abatement worker, lead abatement supervisor, lead inspector/risk assessor and lead abatement planner/project designer shall be issued in writing, bear the Commissioner's signature, and shall contain:2. The date of expiration;3. The name of the permit holder;4. The permit number; and5. A photograph of the permit holder.(c) The permit shall be valid, unless suspended or revoked pursuant to 8:62-3.8, for a period of two years from its date of issuance. N.J. Admin. Code § 8:62-3.6
