Section 8:42-2.2 - Application for licensure(a) Following the acquisition of a Certificate of Need, any person, organization, or corporation desiring to operate an agency shall make application to the Commissioner for a license on form CN-7, Application for New or Amended Acute Care Facility License, prescribed by the Department in accordance with the requirements of this chapter. 1. The application and instructions are available electronically at the Department's "Forms" webpage at http://nj.gov/health/forms.(b) The Department shall charge a nonrefundable fee of $ 2,000 for the filing of an application for licensure and each annual renewal of a home health agency.(c) The Department shall charge a nonrefundable fee of $ 1,000 for the filing of an application for the transfer of ownership of a home health agency.(d) The Department shall charge a nonrefundable fee of $ 250.00 for the filing of an application for the relocation of a home health agency.(e) Any person, organization, or corporation considering application for licensure to operate an agency may make an appointment for a preliminary conference at the Department with the Office of Certificate of Need and Healthcare Facility Licensure.(f) Each home health agency shall be assessed a biennial inspection fee of $ 500.00.1. This fee shall be assessed in the year the agency will be inspected, along with the annual licensure fee for that year.2. The fee shall be added to the initial licensure for new agencies.3. Failure to pay the inspection fee shall result in non-renewal of the license for existing agencies and the refusal to issue an initial license for new agencies.4. This fee shall be imposed only every other year even if inspections occur more frequently and only for the inspection required to either issue an initial license or to renew an existing license.5. This fee shall not be imposed for any other type of inspection.N.J. Admin. Code § 8:42-2.2
