Upon receipt by the Board of a notarized statement from the licensed nursing home administrator that a license has been lost, mutilated, stolen or destroyed, the Department shall issue a replacement license upon payment of the appropriate fee as required at 8:34-9.1(a) and under such conditions as the Board may prescribe.
