N.J. Admin. Code § 7:14B-8.3

For all confirmed releases from an underground storage tank subject to regulation at 40 CFR Part 280, the owner or operator shall report to the Department the source and cause of the confirmed release on a form found on the Department's website at http://www.nj.gov/dep/srp/srra/forms/ in accordance with the timeframe applicable for submittal of the site investigation or remedial investigation report.
