Each permit holder, the account wagering licensee, the off-track wagering licensee, and the fixed odds wagering licensee shall promptly notify the Racing Commission Executive Director, or designee, in writing of any incident which occurs in violation of this chapter or of the internal control procedures implemented pursuant to the procedure set forth at N.J.A.C. 13:74A-5.1.
