Section 13:69I-1.5A - Application for initial Casino Hotel Alcoholic Beverage License(a) An application for an initial casino hotel alcoholic beverage (CHAB) license pursuant to 5:12-103(g) shall consist of the fee specified in 13:69A-9.8 and the following: 1. For a vendor registrant, a completed original and one copy of a CHAB license application form as set forth in 13:69A-5.16; or2. For a casino licensee or applicant, the information required by 13:69C-5.3(a)5.(b) No initial CHAB license application shall be accepted by the Division pursuant to (a)1 above unless the applicant has completed the vendor registration process pursuant to Section 92c. Such application shall include the fee as specified in 13:69A-9.8 and the submission of a CHAB Business Entity Disclosure Form, a CHAB Qualifier Disclosure Form, if applicable, a CHAB Business Entity Disclosure Form-Holding Company and an Equal Opportunity and Affirmative Action Obligation Form.
