Section 13:39-3.4 - Change of address of record; service of process

(a) A pharmacist shall notify the Board in writing of any change in his or her address of record within 30 days.

(b) Failure to notify the Board of any change in a pharmacist's address of record pursuant to (a) above may result in disciplinary action in accordance with 45:1-21(h) and 13:45C-1.3, and the imposition of penalties set forth in 45:1-25.

(c) Service of any administrative complaint or other Board-initiated process at a pharmacist's address of record shall be deemed adequate notice for the purposes of 1:1-7.1 and the commencement of any disciplinary proceedings.
