A third party administrator shall immediately notify the Commissioner in writing of any material change in its ownership, control, or other fact or circumstance affecting its qualifications for a licensure or registration, as applicable, including, but not limited to, a change in name, new or revised standard contracts, or any other information required by this subchapter to be supplied with an initial license application.
