For the purpose of assisting the Disciplinary Hearing Officer or Adjustment Committee in determining the appropriate sanction to impose, each correctional facility shall have available at the hearing a Discipline Record Card for each inmate. This card shall accompany the inmate should he/she be transferred to another correctional facility.
