Section 10A:31-16.3 - Disciplinary report(a) Employees shall prepare a disciplinary report when the employees have reasonable belief that an inmate has committed a violation of the adult county correctional facility rules.(b) The disciplinary report shall include the following information: 1. The specific rule violated;2. Facts supporting the charge;3. Unusual inmate behavior;4. Staff or inmate witnesses;5. The disposition of any physical evidence;6. Any immediate action taken, including the use of force;7. The reporting staff member's signature;8. The date and time the report is prepared; and9. The date, time and name of staff person who delivers the disciplinary report to the inmate. N.J. Admin. Code § 10A:31-16.3
