Section 10A:24-1.5 - Forms

The clinical screening and assessment form related to correctional treatment programs shall be reproduced by each correctional facility/unit from originals that are available by contacting the Office of Substance Abuse Programming and Addictions Services, Division of Operations, within the Health Services Unit.
