Section Spe 604.03 - Documents(a) An applicant for renewal of certification shall submit:(1) A detailed report of the relevant circumstances if any of the answers to questions (e)(2) on part one of the renewal application form is in the affirmative; and(2) On a separate sheet, a detailed report of the relevant circumstances if any of the answers to questions (m) on part 2 of the renewal application form is in the affirmative.(b) The applicant shall not submit documentation of maintenance of continuing competence, but shall retain such documentation in his or her own files for at least 3 years.N.H.
