Section Saf-Mec 804.03 - Required Supporting Material(a) An applicant for a voluntary water treatment initial certification shall provide the following documents:(1) On a separate sheet, a detailed report of the relevant circumstances if any of the answers to the "Yes" no "No" questions on the application listed in Saf-Mec 803.02(a) are in the affirmative;(2) If the applicant holds a certification or other authorization to practice as a water treatment technician or water treatment trainee in another jurisdiction, and unless the information is available only from a secure internet site operated by the issuing jurisdiction, an official letter sent directly to the board by the issuing jurisdiction, stating:a. Whether the certification or other authorization is or was, during its period of validity, in good standing; andb. Whether disciplinary action was taken against the certification or other authorization.(3) Proof of successful completion of the education program described in Mec 805; and(4) Proof of passing the examination at the end of the educational program described in Mec 805.(b) If the applicant is seeking initial certification as a water treatment trainee the individual shall submit a letter from the board-certified water treatment technician that will be providing the training.(c) If the applicant is seeking initial certification after having completed the training not less than 2000 hours within one year of certification the water technician trainee shall submit the following: (1) A signed and dated letter written on personal or business letterhead by a certified water treatment technician who directly supervised the applicant during the period of their training that includes the following:a. The beginning and ending date of the time period(s), not less than 2000 hours completed in one year, during which the applicant was directly supervised by the writer of the letter; andb. The opinion of the writer regarding the competence of the applicant to work independently as a certified water treatment technician; and(2) Evidence of having taken and successfully passed the examination to become a water treatment technician described in Mec 805.(d) If the applicant is seeking initial certification based on having been a water treatment technician in another state the applicant shall submit the following: (1) A letter of good standing from all other states the applicant has ever been licensed or certified as a water treatment technician; and(2) A completed "Affidavit for Proof of Appropriate Field Experience" revised 9/2022.(e) If the applicant is seeking certification on the basis of having operated as a water treatment technician for a least one year prior to January 1, 2024 the applicant shall submit a letter signed and dated by the principal in the business(es) during which the applicant worked as a water treatment technician and includes the following: (1) The beginning and ending date of the time period(s) during which the applicant worked as a water treatment technician for the principal in the business(es); and(2) A statement declaring that for at least one year prior to January 1, 2024 the applicant operated independently as a water treatment technician.N.H.
