Section Saf-C 9408.03 - Form of Request for Hearing(a) Each request for a hearing shall be in writing and contain the following information: (1) Name, address, and telephone number of person requesting the hearing;(2) Make and registration of vehicle in question;(3) Date and time vehicle was removed from its location;(4) Location of vehicle after removal;(5) Name of authorized officer involved, if known; and(6) Reason for contesting the removal.(b) All requests for hearings shall be addressed to: Department of Safety
