Section Saf-C 6510.03 - Content of Monitoring Reports Monitoring reports shall include at minimum:
(a) A statement that the device: (2) Has been inspected and is functioning properly; and(3) Shows no evidence of tampering; and(b) The, date, time, and location of the next monitoring check;(c) The name, location, and telephone number of the interlock service provider;(d) Photos supporting each breath test or attempt within the report;(e) The date and time of each event listed;(f) The name, date of birth, and address of the user;(g) Driver license number and driver license state of the user;(h) The following vehicle information: (1) Make, model, and year;(2) Vehicle identification number;(3) License plate number and state of issuance; and(4) Odometer reading; and(i) Calibration readings of the device;(j) Make, model, and serial number of the device;(k) Dates of the monitoring period;(l) Any comments from the interlock service provider; and(m) Jurisdiction(s) where the user is being monitored.N.H.
